Registering

  • The Practice has a designated practice area agreed with the Primary Care Trust (PCT).
  • A patient wishing to register will be asked for a passport and a current utility bill to verify address and personal details to ensure that they are eligible for NHS care and are living within our area.
  • All new patients will be asked to see a member of staff for a basic health check. This involves completing a health questionnaire, recording of height, weight, blood pressure and a urine test to screen for diabetes and kidney disease.
